Thank you for scheduling your pediatric dental appointment at Dr.J Pediatric Dentistry.
We can’t wait to see you. Before your appointment day, here are a few items to remember.
Check Your Phone
You should receive a text message and/or email from us after your appointment is booked.
New Patients: This text will contain a link to complete your patient paperwork online before you arrive. Your forms can be completed on a computer, tablet, or even your mobile phone.
Returning Patients: You’ll receive your forms link after you confirm the appointment, usually 3-4 days before the appointment.
If you have questions, you can reply to the text message.
Bring Your ID and Insurance Card
Please remember to bring your drivers license or identification with you to your appointment.
As a reminder, we accept most dental insurances, including Medicaid and CHIP. Bring your insurance card. Call or text our office ahead of time if you have questions about your coverage.
Arrive a Few Minutes Early
Your appointment is intended to be a relaxed and postive experience.
Our care team will conduct a thorough examination and/or cleaning, explain any treatment options, and answer any questions you may have.
